Office Policies

Appointments and Office Hours

Our office is open 8:30 a.m. until 5:00 p.m. Monday through Thursday and 8:30am until 4:30pm Fridays, except holidays.

You may make an appointment by calling the office at (502) 935-8061, Monday through Friday, during normal business hours. We will schedule your appointment for the first available opening. When calling to schedule an appointment you will be asked to give a brief description of your orthopaedic problem, how and when it happened, what type of insurance you have and what type is covering this visit. You will also be asked to bring any X-rays, MRI’s, CT scan’s, Myelogram films, lab results or referral information to your first appointment. At times your appointment may be changed due to unexpected emergencies.

We will need to verify insurance information if this visit is related to a motor vehicle accident or a Worker’s compensation injury, prior to your being seen. Please call this information into our front desk at least 2 days prior to your appointment. We will try to get approval and check benefits in advance. Otherwise, this may cause a delay on your visit date, and you possibly may have to reschedule if we can not get approval to treat you.

For Worker’s Compensation, we will need the following:

  • Date of injury
  • Employer Name, address and phone number
  • Worker’s compensation Insurance Co. name, address and phone
  • Claim # and Adjuster’s name
  • In addition, we will need your personal health information. Sometimes treatments are unrelated to the work injury.

For Motor Vehicle Accident injuries, we will need the following:

  • Date of injury
  • Insured name, address and phone number
  • Auto Insurance Co. name, address and phone number for Medical bills
  • Claim # and Adjuster’s name
  • In addition, we will need your personal health information
  • (If a referral is needed we suggest you get one now, in case your benefits run out from the Auto Insurance, otherwise you will be responsible.)

To best care for our patients under the age of 18, an adult or legal guardian must accompany minors.

If you need to cancel or reschedule your appointment, please notify us as far in advance as possible so that your time may be given to another patient in need of care.

What to bring to your appointment

We will need for you to bring to your office visit the following items:

  • Completed forms downloaded from our patient forms page 
  • Current picture Drivers License or Picture ID card
  • Insurance ID card and Co-Pay
  • Any pertinent billing information, other than health insurance (see above Appointment and Office Hours section for specifics)
  • Your referral, from your PRIMARY CARE PHYSICIAN, for insurance billing
  • Any prior Xrays, MRI, CT, Myelogram films
  • List of any medications that you are currently taking (prescribed and over the counter or herbal)

Appointments can be made during calling our office during normal business hours.

After Hours & Emergency Care

If you need to reach a physician after office hours, call the office (502) 935-8061 and our 24-hour answering service will forward your message to the doctor on call to assist you with your problem.

If you have a true emergency, go directly to an emergency room and tell them who is currently treating you for your problem.

New Prescriptions and Refills

All prescription and refill requests must be phoned to our office during normal office hours, Monday through Friday. We invite you to use our automated pharmacy refill line, (502) 935-8061, extension 1280. We now offer Prescription refill requests online on our website. Please click on Prescription refill box to access this feature. Any request after 3:00pm will be processed the next business day. We cannot refill medication after office hours or on the weekend.

The Prescription Agreement
is a contract between you and your physician. This contract is an agreement to prevent the misunderstandings about prescription medicines you will be taking for pain control while being treated by this medical practice.

It should be understood that if you break this contract, your doctor will stop prescribing prescription pain control medications and reserves the right to refuse prescribing pain control medications in the future. As well as, the right to refuse to see you any longer as a patient.  Click here to review The Prescription Agreement

Insurance and Patient Responsibility

Please understand that insurance coverage is an arrangement between your and your insurance company, and that your insurance company may not provide full payment of your medical costs.

We participate in a variety of insurance plans, and these can change frequently. Please check with your carrier to be sure that we participate in your plan. You will be responsible for your bill if we do not participate with your insurance plan.

If your insurance plan will only allow you to see physicians that are members of their network, please verify that we do participate with your particular plan.

If your insurance plan requires you to obtain a referral from your primary care physician, it is your responsibility to obtain this referral prior to your scheduled appointment.

If your insurance plan requires a co-payment, this payment is expected at the time of service.

If you are scheduled for diagnostic testing or surgery, please check with your insurance company to determine if it requires you to obtain a pre-authorization or second opinion.

Payment of the account is the responsibility of the patient. We now offer Billpay online through our website. Please click on "Pay Your Bill" link in the In This Section box  on the left of your screen, to access this feature.

Disability and Insurance Forms

We will complete your disability or other insurance forms. In order to accommodate everyone's needs, we ask that you turn in the forms as soon as possible so that we have sufficient time for completion. We ask that you allow 5-7 business days for your forms to be completed. Please understand that we offer this service to all of our patients and that they are completed in the order in which they are received. There is a charge for completing these forms. Please ask about this fee when sending in your forms.

Picking Up Medical Records and XRays

There are times you may need to obtain a copy of your medical records. To ensure patient confidentiality, you will be asked to sign a release of medical records before allowing anyone to obtain access to your medical information. We must have a signed release from the patient and cannot release records with a telephone request. Fees may apply, be sure to check when requesting records.
